We're excited to welcome you to the YMCA of Silicon Valley as a fitness program through Santa Clara Family Health Plan.
This is offered to Santa Clara Family Health Plan members at no cost to encourage an active lifestyle and improve overall quality of life, and the YMCA is ready to help you reach your health goals. Look around our website to learn how the Y can be your partner in wellness.
To sign-up in person, visit any YMCA of Silicon Valley location and show your SCFHP member ID card. Our Y team will provide a tablet for you to begin the application process. You can also sign up online using the form directly below on this page.
Members under 12 need to have a guardian with them at all times. The guardian does not have to be an SCFHP member or a YMCA member.
Santa Clara Family Health Plan DualConnect is an HMO D-SNP with a Medicare and Medi-Cal contract. Enrollment in DualConnect depends on contract renewal.
YMCA of Silicon Valley is not part of SCFHP. Participation in this program is optional and does not affect your Medi-Cal or Medicare coverage.
